Psychotherapist and psychiatrist: what is the difference

A psychiatrist and a psychotherapist are both doctors. The difference is the instrument: a psychiatrist works with diagnosis and medication, a psychotherapist with conversation. A psychologist is not a doctor at all.

My name is Roman Budaev. I am a doctor and a psychotherapist with 25 years of practice. The question «what is the difference between a psychotherapist and a psychiatrist» is the one I hear most often at a first meeting. Usually it is not curiosity behind it, but fear: a person already feels bad and is afraid of ending up in the wrong place.

Let me go through it in order. Without medical jargon.

A doctor. Makes a diagnosis, prescribes prescription-only medication, refers to hospital when needed. Their territory is severe conditions, where medication cannot be avoided.

Psychotherapist

Also a doctor, with a medical university degree. But treats through conversation: looks for where the condition came from and what keeps it in place.

Psychologist

Not a doctor. Does not diagnose and does not prescribe. Helps you make sense of yourself, of relationships, of decisions.

In short

People go to a psychiatrist when the mind can no longer cope on its own. To a psychotherapist — when the cause needs to be understood. To a psychologist — when a situation needs to be worked through.

The main difference is in the second and the fourth row. A psychiatrist and a psychotherapist are both doctors, both may prescribe, both may diagnose. But a psychiatrist first of all selects a medication, while a psychotherapist first of all works out where the condition came from.

A psychologist stands apart from both. This is not «a cheaper doctor» and not a first step — it is a different profession with different tasks. Confusing them is harmful in both directions: going to a psychologist with real depression makes no sense, and going to a psychiatrist with the question «should I leave this relationship» makes no sense either.

What a psychiatrist can do that a psychotherapist cannot

A psychiatrist has four instruments that nobody else has.

The full range of prescription medication. Antipsychotics, mood stabilisers, serious treatment regimens — that is their territory, and it should not be entered without their training.
Hospital admission. When a person is a danger to themselves or to others, inpatient care is needed. That decision belongs to a psychiatrist.
An official diagnosis with legal consequences. The kind that affects fitness for work, assessments, documents.
Differential diagnosis of severe conditions. Telling bipolar disorder from recurrent depression, schizophrenia from schizoaffective disorder — that is work which requires psychiatric training specifically.

A psychotherapist knows all of this. But works somewhere else — where the mind still copes, though it copes at a high cost.

When you should go to a psychiatrist

The list is short, but worth reading carefully:

  • thoughts of ending your life, or a specific plan
  • voices, visions, a feeling of being watched or of your thoughts being controlled
  • several days almost without sleep and with no tiredness at all, a sense of omnipotence, a flood of ideas
  • depression in which a person has stopped eating, getting up, washing
  • loss of contact with reality after trauma, loss or serious illness

Why this is a medical speciality and not «just talking»

The most common misconception: a psychotherapist is a psychologist who charges more.

No. A medical psychotherapist first completes a full medical education, and only then trains in psychotherapy on top of it. That means reading your test results, understanding how the thyroid works, knowing the side effects of the medication another doctor prescribed you, and seeing the difference between panic attacks and a heart rhythm disturbance.

Conversation is the method, not the absence of one. Like a scalpel for a surgeon: the instrument is the same, and the outcome depends on who holds it and what they understand at that moment.

I look for the cause instead of muting the symptom. We work out where the condition came from and what keeps it in place.

How you become a medical psychotherapist: my own path

Abstract phrasing like «obtains a medical education, then completes a specialisation» explains nothing. Here it is concretely, on my own example.

1996–2002

Kharkiv National Medical University. Six years of general medicine: anatomy, physiology, internal medicine, surgery, hospital shifts.

2003

Internship in paediatric neurology, KhMAPE. The nervous system at close range. That is where my understanding of how the bodily turns into the mental, and back, comes from.

2007

Specialisation in psychotherapy, KhMAPE. Separate training on top of the medical degree.

Between neurology and psychotherapy there were more than ten years of work with the body. More on that below: it still shapes the way I work.

And who is a psychologist, and how do they differ from both

A psychologist graduates from a faculty of psychology, not from a medical university. They are not a doctor. They do not diagnose, do not prescribe and do not treat disorders — this is not nitpicking about terms, these are the boundaries of the profession.

What a psychologist does well: helps you make sense of relationships, self-esteem, choices, conflict, living through a loss. Supports you when things are hard but the mind is coping.

Separately there is the clinical psychologist, the one most often confused with a psychotherapist. This is a psychologist with additional training who works alongside doctors and carries out psychological assessment. But that does not make them a doctor, and they will not write a prescription.

How to tell you have come to the wrong specialist

If a state has lasted for months, does not depend on what happens around you, and comes with disturbed sleep, appetite or weight, or with thoughts of «what is all this for» — the question is already a medical one. A good psychologist will say so themselves, and that is a sign of a conscientious specialist.

About medication

Can a psychotherapist prescribe medication

Yes. A medical psychotherapist has the right to prescribe — the degree allows it.

The question is different: do they use that right.

In the overwhelming majority of cases I work without medication. Not out of principle and not out of ideology — simply because a tablet removes the symptom but does not answer the question of where it came from. A person mutes anxiety for two years, then stops the drug, and the anxiety comes back to exactly the same place. Nobody touched the cause.

There are conditions where medication is needed, and I will say so plainly. Severe depression, acute states, situations where psychotherapy will simply not start without medical support. In those cases I refer to a psychiatrist and do not consider it a defeat.

If it is still unclear after the table, that is normal. Describe the situation in two or three sentences and I will tell you whether it is my case. If it is not — I will name who to go to and will not take it on.

The border

Psychosomatics or a bodily illness: where the border runs

There is a separate group of people who reach me: those who have already been through every doctor.

Blood tests, an MRI, an ECG, a cardiologist, a gastroenterologist, a neurologist. Everything clean. In the end they hear something like «it is your nerves» and stay exactly where they started, only with a folder of test results.

At an appointment: conversation is the main instrument of a medical psychotherapist

Before working with the mind, I worked with the body for more than ten years. That is why I see the difference between psychosomatics and an illness that has to be treated by the relevant specialist.

What I look at first:

Is there a link to a situation. A symptom that appears in specific circumstances and goes away outside them is more likely psychosomatic. One that depends on nothing needs a specialist doctor.
How it behaves at night. Symptoms that wake you in the middle of the night and are not linked to anxious waking need the attention of physical medicine.
The pattern over time. Psychosomatics stays at the same level for years or migrates. Organic pathology usually grows.
What the tests actually showed. Not «clean» as a word, but which examinations were done and when.

This does not replace examination. It helps to understand which way to go next.

Referral

Do you need a referral to get an appointment

No referral is needed for a private appointment. Neither with me nor with any other doctor in private practice. No medical card, no forms, no queue. A message is enough.

In the state system the order is different: as a rule the route starts with a family doctor who issues a referral to a specialist. The rules and the scope of free care change, so it is worth checking the current conditions with the institution you plan to approach.

About «being put on the register». This is the most common fear and the most common reason people wait for years. The rules for follow-up in state institutions have changed more than once in recent years — if this matters to you, ask about it directly, before any examination begins.

Frequently asked questions

Can I go straight to a psychotherapist, skipping the psychiatrist?

Yes. In most cases that is where to start. If at the first meeting I see that a psychiatrist is needed, I will say so plainly and explain why.

Are a psychotherapist and a psychoanalyst the same thing?

No. Psychoanalysis is one of the methods within psychotherapy, not a separate profession. I work in that approach among others, but I am not limited to it.

How much does an appointment cost?

A first consultation is 500 UAH for 55 minutes. Online is the same. Follow-up sessions are agreed individually and I say the price before you book.

What if I come and realise it is not for me?

Then it is not for you. At the first meeting I will honestly say whether this is my case. If it is not, I will not take it on and will name who to go to.

Will anyone find out that I came?

No. Not your employer, not your relatives, not a clinic.

Is online treatment possible?

Yes, the format works. An online appointment runs the same 55 minutes and costs the same. My patients come from 19 countries.
